Be sure to attach a void cheque (or copy of a void cheque).
If Applying for Assured Access, Medavie Blue Cross will require 12 months claims history from your existing group insurer or a signature on the application from your family physician.
If applying for Medavie Blue Cross Non Medical Life Insurance, a cheque for the first month’s premium is required.
